New Patient Application
Vital Health is now welcoming new patients, from young adults to seniors looking for everything from routine care to specialized medical attention.
- Your initial appointment will last approximately 30 minutes. For your first visit to Vital Health, please arrive at least 15 minutes early. Remember to bring your health insurance card, a photo ID, and any medication bottles you are currently taking.
- You can download and print the necessary forms below to complete the new patient paperwork in advance. Once completed, you can drop off the forms in person, or email it to our office. After receiving your paperwork, our office administrator will reach out to schedule your appointment. You can also complete our New Patient Questionnaire HERE.
